Buerger’s Disease Overview

Buerger’s disease, also known as Thromboangiitis obliterans, is a rare vascular disease in which blood vessels of the hands and feet become blocked. Affected individuals may notice pain as an early finding, or coldness in the fingers; later findings include fingers or toes that appear pale, red, or bluish, sudden severe pain in the hands and feet, skin ulcers, gangrene, and, ultimately, amputation. Diagnosis of the vessel blockage can be made through plethysmography, Doppler ultrasound of the extremity, or catheter-based x-ray arteriogram, and blood tests may be performed to rule out other causes of vasculitis or blood vessel occlusion.

The goal of treatment for Buerger’s disease is to control symptoms and prevent the disease from worsening. Total tobacco cessation — including smoking, secondhand smoke, and nicotine replacement therapy — is the only treatment shown to improve symptoms and reduce the risk of amputation, provided it occurs before the onset of gangrene or tissue loss. Additional medication options include calcium channel blockers, anticoagulants, thrombolytics, and prostaglandin analogs, alongside interventions such as sympathectomy, adrenalectomy, spinal cord stimulators, and omental transfers.

Buerger’s Disease Emerging Drugs Profile

  • Mesenchymal Stem Cell Therapy (VascoStem): K-STEMCELL This mesenchymal stem cell therapy is a drug candidate that improves or treats ischemic ulcers and other complications caused by Buerger’s disease. Once injected, VascoStem indirectly vascularizes tissue through the vascular endothelial growth factor (VEGF) it secretes and directly restores damaged tissue. The molecule is in the Phase I/II clinical trial stage for the treatment of Buerger’s Disease. VascoStem was awarded rare pharmaceutical product designation by the U.S. Food and Drug Administration (FDA) in May 2016 as a treatment for Buerger’s disease, based on the results of Phase I/II clinical trials in Korea, and the European Medicines Agency (EMA) granted it orphan drug designation in March 2017.

  • ATI-1013: Antidote Therapeutics ATI-1013 is a human monoclonal antibody that sequesters nicotine in the blood, discovered by harnessing the human immune system’s capacity to generate high-affinity antibodies to external antigens. The nicotine-specific mAb program has been supported by an $8.5 million Strategic Alliance grant award from the National Institute on Drug Abuse Division of Therapeutics and Medical Consequences at the National Institutes of Health, which led to the discovery and development of ATI-1013. The drug has also received Orphan Drug Designation from the FDA.
